Technical Assistance:
Today we discussed inspection results, medication, transportation, documentation of developmental skills and labeling lunches.

Comments:
An unannounced monitoring inspection was conducted today between 9:50 and 1:30pm. There were 51 children in care with seven staff. Children and staff records were reviewed. A walking tour inside and outside the center was completed. Required inspections and posting were reviewed. Children were observed in teacher and self-directed activities including but not limited to lunch, transitions, nap, hand washing and restroom routines. Violations in the areas of administration, physical plant, special services, special care and emergencies and background checks were observed and may be reviewed on the violation notice.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review and interview the center failed to maintain children's records as required. Evidence: 1 - The record for child 1 has a documented first date of attendance as 8/31/15. However, there is not documentation the information was updated annually as required. 2 - Staff 1 confirmed that there is not documentation of information update in the child's record.

Plan of Correction: Center management will create and implement a system for ensuring that each child's record is updated annually as required.

Standard #: 22VAC40-185-280-B
Description: Based on observation and interview the center failed to ensure that hazardous substances are kept in a locked location. Evidence: 1 - Staff 3 stated that the kitchen door is not locked when children are in care. However, there is bleached stored in a kitchen cabinet under the sink. 2 - There are three cabinet doors that allow access to the location of the bleach. One of the three doors has child proof that is not working. 3 - Staff 3 confirmed that the cabinet child proof latch did not work

Plan of Correction: Center management will replace or repair the broken latch.

Standard #: 22VAC40-185-500-B
Description: Based on observation and interview the center failed to ensure that a nonabsorbent surface is used for diapering or changing for children younger than three years. Additionally, the center failed to ensure that the surface is cleaned with soap and water and sanitized after each use. Evidence: 1 - The diaper changing in the two year old child care room has seams that have come apart in several places. Thereby exposing the foam fill and creating an absorbent where a nonabsorbent surface is required. a. Staff 4 confirmed that diaper changing mat has exposed foam fill. 2 - Staff 4 reports that pull-ups are changed in front of the toilet and not on the changing table. 3 - Staff 4 reports that she uses a Clorox wipe to clean and sanitize the diapering surface after each diaper change. a. The staff was not aware that the diaper changing process was a two step process including the need for the surface to be sprayed or dipped with sanitizer and allowed to air dry.

Plan of Correction: Will retrain staff regarding diaper changing requirements. Additional mats will be ordered.

Standard #: 22VAC40-185-560-F
Description: The center failed to ensure that nutritional requirements are followed from a recognized authority such as USDA. Evidence: Staff 3 was unable to answer questions regarding the amount of required foods served to children for lunch

Plan of Correction: Center management will provide the USDA portion chart to the staff along with proper measuring utensils.

Standard #: 22VAC40-191-40-D-1-C
Description: Based on record review and interview the center failed to ensure that background checks are updated as required. Evidence: 1 - The record for staff 5 contains a central registry background check that is dated 4/10/13. Therefore, the background check is past due for update. 2 - Staff 1 confirmed that the background check is pasted due for update.

Plan of Correction: Center management requested an updated background check on 10/13/16. Going forward updated background checks will be requested prior to expiration.
