An unannounced monitoring inspection was conducted on 11/15/16 from 5:00pm - 6:35pm. At the time of entrance there were 42 school age children in care with eight staff. Children were observed eating snack, completing homework, and participating in various activities in the cafeteria, gym and classrooms. Records were reviewed for four children while at the facility. The records for five staff were reviewed on 11/17/16 at the central office. Medication, emergency procedures and emergency supplies were reviewed during the inspection. Areas of non-compliance are identified on the violation notice and were discussed during the exit interview.
Standard #: 22VAC40-185-70-A Description: Based on a review of documentation and interviews, it was determined that the facility did not ensure that they maintain and keep at the center a complete record for each staff that contains all required information. Evidence 1. At the facility, there was no documentation for the name, address, and telephone number of a person to be notified in an emergency for five of ten staff working during the inspection. 2. Staff #1 (Program Director) confirmed the emergency contact information for all staff was not available at the facility during the inspection. Plan of Correction: I will obtain the staff emergency contacts located at the Administrative Office from the Service Director, Reggie Carter. After getting a copy of each document I will ensure that they are placed in the licensing binder located at the club-site.
Standard #: 22VAC40-185-260-A Description: Based on a review of documentation and interviews, it was determined that the facility did not ensure that an annual fire inspection report from the appropriate fire official having jurisdiction was completed. Evidence: 1. The most recent fire inspection that was available for viewing during the inspection was dated 10/14/15. 2. Staff #1(Program Director) confirmed the annual fire inspection for the current year had not been completed. Plan of Correction: I will communicate with the John F. Kennedy Middle School and remind them that the fire inspection is overdue. Each Monday I will check in with them to ensure that they remember to communicate with the fire marshal.
