An unannounced monitoring inspection was conducted from 7:20 am - 9:00 am. There were 26 children in care being supervised by three staff members. The children were observed participating in cooperative games and craft activities. Breakfast served today is french toast sticks, cereal, fruit juice and milk. Ten children's records were reviewed and in compliance. Three staff records were reviewed with areas of noncompliance documented on the violation notice. Nine mediations were reviewed. There is currently one staff member on site who is MAT (Medication Administration Training) certified. The most recent emergency escape drill and shelter in place drill was conducted 11/8/2016. Standard #: 22VAC40-185-240-A Description: Based on review of three staff records it was determined that not all staff obtained documentation of orientation training on file by the end of the first day of assuming job responsibilities. Evidence: Staff A with a start date of 8/17/2015 did not have documentation of orientation training on file. Plan of Correction: Administration will request the signed copy of the completed orientation training document from the corporate office.
Standard #: 22VAC40-185-510-N Description: Based on review of nine medications it was determined that not medication was sent home or disposed of within 14 days of the expiration of the medication authorization. Child A had two emergency medications on site with authorization dates of 11/02/2015. Plan of Correction: Administration will have the parents of child A renew the medication authorization forms.
Standard #: 22VAC40-191-60-B Description: Based on review of three staff records it was determined that not all employees obtained a signed sworn statement or affirmation before the first day of employment. Evidence: Staff A with a start date of 8/17/2015 did not have signed documentation of a sworn statement or affirmation. Plan of Correction: Administration will receive a copy of the sworn statement from the corporate office.
