Comments:
An unannounced monitoring inspection was conducted from 10:30 am - 12:30 pm. There were 79 children in care being supervised by 13 staff members. The children were observed participating in structured center play and table top activities. Lunch served today is soynut butter and jelly sandwich on whole grain bread, green beans, peaches, and milk. Ten children's records were reviewed and in compliance. Seven staff records were reviewed. The most recent emergency escape drill was conducted 11/15/2016 and the most recent shelter in place drill was conducted 11/16/2016. Four emergency medications were observed and in compliance. Description: Based on review of seven staff records it was determined that not all background checks were repeated every three years. Evidence: The most recent Central Registry Check on file for Staff A was dated 10/03/2012.

Plan of Correction: Administration will submit the Central Registry Check for Staff A today.
