Comments:
An unannounced renewal inspection was conducted today from 11:55am-1:45pm. There were 91children (infants-5yrs.) directly supervised by 15 staff. The physical plant, 4 staff records, 4 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in, reading books, playing, eating lunch and preparing for rest time. Diapering and hand-washing procedures were also observed. There was an abundant supply of materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice. Description: Based on review, child records were incomplete. Evidence: Child #4 did not have health policy number

Plan of Correction: Mom was e- mailed during inspection

Standard #: 22VAC40-185-270-A
Description: Based inspection of Child Day Center all areas were not safely maintained. Evidence: closet in toddler room door was chipping, paint was chipping on the wall in 2's room and baseboard falling off in pre K room.

Plan of Correction: Our center will undergo renovations where all repairs will be addressed.

Description: Based on inspection, medication on site was not labeled with child name and prescription label. Evidence: Child #6's medication did not have a prescription label.

Plan of Correction: Director e-mailed mother during inspection.

Standard #: 22VAC40-185-550-D
Description: Based on review of documentation and interview with director an evacuation drill was not conducted for October 2016.

Plan of Correction: Will do an extra drill this month
