The licensing inspector conducted an unannounced monitoring inspection Monday, October 17, 2016 from 4:14pm to 5:42pm. The children were observed arriving to the center from school, playing outside on the playground and eating afternoon snack. The infants were observed playing on the floor with their teachers. The staff was observed interacting with the children, helping them get off of the bus and get snack. All classrooms and playgrounds were inspected today. Three children?s records and three employee records were reviewed during this inspection. The center administers medication; however they did not have any being given at this time. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 10/13/2016 Last shelter-in-place drill: 8/18/2016 Last fire inspection: 4/1/2016 Last health inspection: 8/3/2016 Today, the following child to staff ratios were observed: Infants ?5:2 Twos ? 12:3 Threes ?9:2 Pre-K ?10:1 School Age (K-1st grade) ?19:1 School Age ?13:1 The violations from the previous inspections were checked for corrections. There was one repeat violation found during today?s inspection. If you have any questions about this inspection, please contact the licensing inspector, Roshon Casey, at (804) 662-9760. PROVIDER: Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 calendar days from today. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Standard #: 22VAC40-185-330-B Description: Based on observation on 10/17/2016, the center did not ensure that where playground equipment is provided the resilient surfacing complied with minimum safety standards. EVIDENCE: The pea gravel on the preschool playground with the sliding equipment measured 4.5 inches in depth. The minimum required depth is 6 inches. Plan of Correction: Not available online.
