1. Electronic files are an acceptable method for storing general records: However, if electronic files are used, provisions must be in place to address ?securing essential documents? in an emergency as required by 22 VAC 40-185-550.B.3.b (securing essential documents for evacuation emergencies) and 22 VAC 40-185-550.B.4.b (securing essential documents for shelter-in-place emergencies). 2. To assure that the required amount of resilient surface is maintained, it may be necessary to rake the resilient surface after each playground use. Having a system for regular raking is important since children have been seriously injured from falls onto an inadequate resilient surface. This maintenance of resilient surfacing could also be a learning experience for children by having them help rake the resilient surface with child size equipment. In addition to raking, ?wear? mats are also acceptable in combination with a loose fill resilient surface under swings or at the bottom of slides to help assure a safe surface.
The licensing inspector conducted an unannounced monitoring inspection Monday, November 21, 2016 from 1:27pm to 3:29pm. The children were observed resting on mats. The infants were observed playing with toys, sleeping and being comforted. A formal observation was completed with this group. Interviews were held with staff throughout the inspection. The staff was observed watching the sleeping children, feeding the babies, and comforting them as well. Some of the school age children arrived at the end of the inspection. All classrooms and playgrounds were inspected today as well as the facility?s mini buses. Three children?s records and seven employee records were reviewed during this inspection. The center administers medication; however they did not have any being given at this time. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 11/15/2016 Shelter-in-place drills: 02/24/2016 & 06/23/2016 Last fire inspection: 07/22/2016 Last health inspection: 01/28/2016 Total enrollment: 80 Today, the following child to staff ratios were observed: Infants ?12:3 Toddlers ?5:1 Twos (rest time ratio) ? 12:1 Preschool & Pre-K (rest time ratio) ?18:1 School Age ?11:1 The violations from the previous inspections were checked for corrections. There were no repeat violations found during today?s inspection. If you have any questions about this inspection, please contact the licensing inspector, Roshon Casey, at (804) 662-9760. PROVIDER: Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 calendar days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.
Standard #: 22VAC40-185-160-A Description: Based on a review of staff records on 11/21/2016, the center did not ensure that one of seven staff records contained documentation of a negative tuberculosis (TB) screening within 12 months prior to employment. EVIDENCE: The TB screening in the record of Staff #4, whose start date was 4/18/2016, is dated 10/23/2014. Plan of Correction: Going forward, we will ensure that TB tests are within a year. This employee now has current up to date TB
Standard #: 22VAC40-185-270-A Description: Based on observation on 11/21/2016, the center did not ensure that all areas of the center inside and outside were maintained in a safe condition. EVIDENCE: There were three broken coat hooks in the School Age 1 classroom. Plan of Correction: Hooks have been taken down and replaced.
Standard #: 22VAC40-185-450-A Description: Based on observation on 11/21/2016, the center did not ensure that mats used by children during the designated rest period shall have linens consisting of a top cover and a bottom cover or a one-piece covering which is open on three edges. EVIDENCE: During rest time five Preschool and Pre-K children in the Pre-K room did not have a top cover. Plan of Correction: Corrected 11/21/2016. We had extra blankets for these children.
Standard #: 22VAC40-191-60-C-2 Description: Based on a review of employee records on 11/21/2016, the center did not ensure that a central registry finding was obtained within 30 days of employment. EVIDENCE: The central registry findings of employee #2, whose date of hire was 4/25/2016, is dated 09/30/2016. Plan of Correction: Going forward, make sure that we mail and date the CPS check within 30 days.
