Comments:
An unannounced monitoring inspection was conducted on December 2, 2016. The director arrived shortly after the inspection began. There were 2 toddlers and 1 infant in the Infant/Toddler Room. The infant was in a swing, and the toddlers were playing with toys. In the Two Year Old Room there were 8 children with 1 staff. Some of the children were playing with toys and the others were doing an art project. There were 4 children with 1 staff in the Three/Four Year Old Room. The children were playing on the playground and then came inside for lunch. Lunch for the day was hot dog with bread, green beans and carrots, fruit, and milk. Four children's files and 2 staff files were reviewed. There were no medications being given. Evacuation and shelter drills had been documented. It was noted that the owner had purchased astro-turf for the playground with extra cushioning in the fall zones of fixed equipment. The installer had supplied documentation of the installation details. Two large canopies had been installed to provide shade. Inspection began: 10:45 Inspection ended: 11:55

Standard #: 22VAC40-191-40-D-1-C
Description: Based on a review of 2 staff records, the center had not obtained background clearances before three years from the dates of the last background clearances on file for one staff. Evidence: The clearances for Staff # 1 had not been obtained before three years since the last clearances were completed. The sworn statement on file was dated 9/3/13, the central registry finding was dated 9/23/13, and the criminal history record was dated 9/30/13.

Plan of Correction: Clearances were requested in November. The center will follow up if clearances are not back within 30 days.
