39 Children were in care during the inspection. Children were observed during free play, snack, and outside time. The age range of the children was four months to nine years old.
Seven staff and ten children's records were reviewed.
13 Staff are current in first aid and CPR.
Four staff are current in MAT.
Inspection time 2:00pm- 5:30pm
Standard #: 22VAC40-185-160-C Description: Based on review of six staff records not all had repeat test with in two years since the last test. Evidence: Staff B's TB test is dated 10/9/13. Plan of Correction: TB test completed for missing staff member.
Standard #: 22VAC40-185-210-A-1 Description: Based on observation and staff interviews not all program leaders are qualified. Evidence: Staff A that was alone supervising seven two year old did not meet program leader qualifications. Staff A is supervising from 4:30pm to 6:30pm Plan of Correction: A lead qualified staff member will be in all rooms at all times.
Standard #: 22VAC40-185-270-A Description: Based on observation areas of the playground were not maintained. Evidence: There are tree roots sticking up where the children run posing a tripping hazard. There are 11 nails sticking out on the fence at a child's level. There are bolts sticking out on the table posing a scraping hazard to the children. The white fence on the playground has peeling paint. Plan of Correction: Work order placed to correct roots sticking up. nails were driven properly into fencing or removed of not needed. work order placed to correct bolts sticking out of picnic table. Paint will be scraped/repainted.
Standard #: 22VAC40-185-280-B Description: Based on observation not all hazardous substances were kept in a locked place. Evidence: In the Pre-K room there was an unlocked cabinet with Spray air fresher, bleach water and disinfectant spray stored with in reach of the children. Plan of Correction: Staff member immediately locked cabinet. All staff will keep cabinets locked at all times.
