Comments:
An unannounced monitoring inspection was conducted today, December 6, 2016 to determine the center's compliance with licensing standards. The inspector arrived at 1:00 pm and concluded the inspection at 3:15 pm. Upon the inspectors arrival, children were observed in a variety of activities. Children in lower elementary were practicing the song "All Ya need is Love" for their upcoming Christmas program. Upper elementary children were having lunch, reading and working on individual projects. Upstairs the two primary classrooms were observed working on individual task. Children were tracing bird cutouts, identifying animals and writing the names on paper, and matching words to pictures. Children in the primary classrooms later prepared for nap. Soft music played in the background. Lower elementary later worked o maps, adding jewels to represent the capital. Families provide their own meals. The facility has age and stage appropriate materials and supplies. The facility had the following posted: License, last inspection, emergency numbers, evacuation maps/procedures, daily schedule and various parent information. The first aid kit and non-medical emergency supplies were inspected and found complete on both floors of the facility. The playground was not inspected due to heavy rain. The inspector interacted with staff and children in care. The following ratios (staff to children ) were observed in each classroom: Lower elementary: (6-8 years): 29 children with 2 staff. Upper elementary: (9-12 years) 23 children with 2 staff. Primary I: (2.5-6 years) 20 children with 2 staff. Primary II: (2.5-6 years) 13 children with 2 staff while napping. The last evacuation drill was conducted on 11/07/2016. A shelter-in-place drill for 2016 year was conducted on 03/07/2016. A second drill will need to be conducted by the end of the year. The facility has current liability Insurance. Health Inspection: 01/11/2016 Fire Inspection: 11/07/2016 Medications on site was reviewed. Two staff have current EMAT certification. Staff have current CPR & First aide training along with Daily Health Observation training. The violations from the previous inspection on April 27, 2016 were checked for corrections. Standard #: 22VAC40-185-160-C
Description: Based on review of staff records, the facility did not obtain documentation of a negative tuberculosis screening at least every two years from the date of the initial screening. Evidence: Staff #1 had a tuberculosis screening dated 07/19/2014, exceeding the two year allowance to renew.

Plan of Correction: Per the Director: I will send the staff today to obtain a tuberculosis screening.

Standard #: 22VAC40-185-350-C
Description: Based on observation and interview, the facility did not maintain the staff to children ratio applicable to the youngest child in an ongoing mixed age grouping. Evidence: 1. There were 29 lower elementary children, aged 6-8 years with two staff in the main classroom. An additional child who appeared to be younger than the lower elementary children was observed on a nap mat in the main classroom. When questioned about the child on the mat, the Director stated that the child was three years old and from the primary classroom. The three year old changed the required ratio to one staff for every ten children.

Plan of Correction: The Director took the three year old back to the primary classroom. Per the Director: We will do additional training regarding nap time ratios.

Standard #: 22VAC40-185-510-E
Description: Based on review of children's medications, the facility did not ensure all medications were labeled with the child's name, the name of the medication, the dosage amount and the time or times to be given. Evidence: The medication of for child #5 was labeled with the child's name but did not have the name of the medication, the dosage amount, and the time or times to be given as required.

Plan of Correction: Per the Director; I will contact the parent and have them provide the original prescription box with all the required information listed.

Standard #: 22VAC40-191-60-C-2
Description: Based on review of staff records, the facility did not obtain a central registry finding within 30 days of employment or volunteer service. Evidence: Staff #2, hired 06/24/2016, did not have a completed central registry finding on record. A central registry had been requested and returned to the center for missing information. The request had not been resubmitted.

Plan of Correction: Per the Director: I will mail the request for the central registry off today.
