An announced initial inspection was conducted Wednesday, December 7, 2016 in response to transfer in ownership. Two representatives from the corporation were present during the inspection. During the inspection, the licensing inspector observed 18 children in care being supervised by two current staff members. The facility plans to operate Monday through Friday and in conjunction with the public school calendar. The facility will service up to 18 Head Start children, three through five years of age. During the initial study, the facility was observed to be equipped with a variety of age appropriate equipment and supplies including materials for gross motor, art, literacy, and dramatic play. Appropriate child size furnishings, including tables, chairs and cots were observed. The Head Start classroom is located in a module unit in the rear of Lancaster Elementary School. A bathroom for the children to use is located in the Head Start classroom. Children have access to the school cafeteria, gym and additional bathrooms. All areas were checked during the initial inspection. The play ground was inspected during the inspection. During the inspection, the emergency supplies including first aid kit, flashlight and battery operated radio were checked. The licensing inspector discussed required documentation for staff and documentation of updated approval from other agencies including fire and health departments with the corporate representatives. Last documented health inspection: 03/07/2016 last documented fire inspection: 05/09/2016 Staff and children records will be reviewed at the next inspection. Though, some standards of care could not be determined during the initial study, the center is in substantial compliance with the standards that could be determined. A conditional (6 month) license will be recommended. An unannounced follow up inspection will be conducted within 60 days to determine compliance with standards not determined at the initial inspection. Standard #: 22VAC40-185-270-A
Description: Based on observations, the facility did not maintain all areas and equipment of the center, inside and outside, in a clean, safe and operable condition. Evidence: A metal latch was observed protruding approximately two and a half inches out from one side of a bookcase used to house toys. At the other end of the bookcase a metal hinge approximately 10 inches was folded in however was easily folded out posing the risk of snagging the skin of a child.

Plan of Correction: Per the corporate representative: We will remove the hinge and latch from the bookcase.

Description: Based on observations, The facility did not ensure cleaning materials and insecticides/pesticides were stored in areas physically separate from each other. Evidence: In the locked closet, cleaning materials and two canisters of insecticide were observed on a shelf.

Plan of Correction: Per the corporate representative: We will review with staff the regulation to keep separate and correct it on the spot.
