An unannounced monitoring inspection was conducted today, December 12, 2016 to determine the center's compliance with licensing standards. The licensing inspectors arrived at approximately 9:30am and concluded the inspection at 11:45 am. The children were engaged in a variety of activities upon the inspectors arrival. Some toddlers were painting using water colors while the remaining toddler rode small trikes around the room. Staff sang Christmas songs and encouraged children to sing as well. A half day preschool program has been added. The class runs in conjunction to the public school calendar. Children meet in the school age classroom while the school agers are attending class at the elementary school. Today children were observed playing in centers and reviewing the letter "E" for elf. Children decorated elf hats and then attached a photo of their face. The preschool and pre-k classrooms played in centers (magnets, play dough, sensory table, dress-up) and worked on art projects (Christmas cards, lace doilies snowman, gift bags for parents). Upper & lower nursery listened to a Christmas story and danced to music while playing with colorful scarfs. The Jr. Kindergarten class is hosting a Christmas Formal Snack later in the week. Children read the "Gingerbread Baby" and plan to build gingerbread houses. Classrooms were decorated with seasonal decorations and children's art work. Staff cleaned the tables and hand washing was conducted before children sat down for snack. Staff interacted, encouraged children and assisted with activities. The inspector interacted with children and staff during the inspection. The classrooms have a variety of age and stage appropriate materials that are in operable condition. All classrooms were inspected to include the front entrance, and kitchen. The play grounds were inspected. A children's garden has been created. Children are growing corn and tomatoes. The center had the following posted: license, daily schedule, emergency numbers, evacuation maps, menu and various parent information. The first aide kit and the emergency supplies were inspected and found to be complete for the building. The play ground was not inspected today due to inclement weather. Three employee records and five children's records were reviewed during today's inspection. Medication is being administered. Medication and medication authorizations were reviewed. There are multiple staff with current MAT training. The majority of staff and all van drivers have current CPR, First aid and daily health observation training. Current Health Inspection: 02/08/2016 Current Fire Inspection: 09/28/2016 The last emergency evacuation drill was conducted on 11/08/2016, The two required shelter-in-place drills for the 2016 year were documented on 02/17/2016 and 11/11/2016. Today the following ratios were observed: half day pre-school program: 5 children with 2 staff. Toddler Room: 6 toddlers with 2 staff. Upper Nursery: (2 years) 6 children with 1 staff. Lower Nursery: (2-3 years) 6 children with 2 staff. Pre-K: (4-5 years): 13 children with 3 staff. Pre-school: (3-4 years) 11 children with 3 staff. Jr. Kindergarten: 7 children with 2 staff. School age: Not in attendance during inspection. The monthly menu was posted and today's lunch consisted of: chicken patty on a bun, mixed vegetables, applesauce and milk. Description: Based on observations, the facility did not ensure all hazardous substances such as cleaning materials, insecticides, and pesticides were kept in a locked place using a safe locking method that prevents access by children. Evidence: A canister of disinfectant, two bottles of toilet bowl cleaner and one bottle of drain clog remover were observed on a wall shelf in the bathroom of the upper nursery classroom. The shelf was approximately six feet from the floor. The cleaning materials were not locked as required.

Plan of Correction: The cleaning materials were removed and locked upon notification. Per the Director: The teacher was notified of the requirement for the future.
