Technical Assistance:
Please ensure that staff engage in active supervision of children at all times, especially while outside.

Comments:
An unannounced renewal inspection was conducted today from 1:30 - 3:45. The children were napping when the inspector arrived. The physical plant was checked throughout the building. There were 6 classes in session. The correct ratios were being met in all classrooms. The children were observed getting up from nap, eating snack, playing with toys, singing songs and playing outside. The exit interview was conducted with the director. The violation notice will be e mailed for completion.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on a review of children's records, the licensee did not obtain all of the required information for three children. Evidence - The record for child #2 did not contain 2 emergency contact addresses, permission for emergency medical treatment and place and telephone number of parents employment. The record for children #4 & 6 did not contain addresses for 2 emergency contacts.

Plan of Correction: All records were updated with the missing information.

Standard #: 22VAC40-185-70-A
Description: Based on a review of staff records, the licensee did not obtain all of the required information for one staff member. Evidence - The record for staff #1 did not contain references.

Plan of Correction: Two references were completed 8/18/16. The paperwork was in another staff file.

Standard #: 22VAC40-185-400-B
Description: Based on observation, the staff in the 2's room did not use constructive behavioral guidance. Evidence - The teacher in the 2's room took away a child's drinking water "for playing with it" and the teacher said "she lost her water".

Plan of Correction: The child was offered more water and the teacher was reminded to use more constructive behavioral guidance.

Standard #: 22VAC40-185-430-L
Description: Based on observation, the staff in the Infant 1 room were using a walker. Evidence - One child was observed being placed in a walker which was part of the equipment in the classroom.

Plan of Correction: The walker was removed.

Standard #: 22VAC40-185-500-A
Description: Based on observation, the teacher in the pre-school 1 room did not ensure that one child washed hands after eating snack. Evidence - One child was observed finishing his snack last and he never went to wash hands before going outside even though he was instructed to do so.

Plan of Correction: The staff was retrained to make sure all children was [sic] their hands after eating.

Standard #: 22VAC40-185-500-B
Description: Based on observation, the staff in the toddler room did not keep the diaper changing area free from all items except diaper changing supplies. Evidence - There was a thermometer observed on the diaper changing area in the toddler room.

Plan of Correction: Diaper changing area was cleared.

Standard #: 22VAC40-185-510-N
Description: Based on a review of children's medications, the licensee did not return all medications to parents 14 days after the authorization end date. Evidence - There were two medications on site that were not returned to parents or discarded within 14 days of the authorization end date.

Plan of Correction: Medications were returned.

Standard #: 22VAC40-185-550-M
Description: Based on a review of the facility's injury report form, the licensee does not obtain all of the required information. Evidence - The facility's injury report form does not include the method or date the parent was notified of the injury.

Plan of Correction: Staff and management will insure [sic] the method and date are added until the form is corrected.

Standard #: 22VAC40-185-560-N
Description: Based on observation, the staff in the toddler room did not prevent one child from walking around while drinking. Evidence - One child was observed walking around while drinking from a sippy cup and then lying down on the floor with the cup before the teacher took it away.

Plan of Correction: Staff was retrained to prevent children from walking around while drinking.

Standard #: 22VAC40-191-40-D-1-A
Description: Based on a review of staff records, the licensee did not obtain a Criminal Record Check from the Virginia State Police for one staff within the required time frame. Evidence - The record for staff #1 hired on 8/22/2016, did not contain a Virginia State Police Criminal Record Check within the required time frame of 30 days.

Plan of Correction: Criminal record was received.
