22VAC40-191 Background Checks (22VAC40-191)
An unannounced monitoring inspection was conducted that included a tour of the facility and a review of records. A sample of 4 children and 4 staff records were reviewed. Children were observed during lunch and nap time. Children were also observed during toileting.
Standard #: 22VAC40-185-270-A Description: Based on observation the center failed to ensure areas and equipment of the center were maintained in a safe condition. Evidence: 1. There was a hole about 1" wide at the bottom of the wooden half door dividing the 2 year-old room and the school-age room. 2. In the early preschool room there was uncooked macaroni in the opened sensory table. The youngest child in the room at the time of inspection was 2 years-old. Due to the size, shape and consistency of the sensory materials, a choking hazard was posed for the children under the age of 3 years. Plan of Correction: . Work order was placed the day of the inspection to repair the hole. Safety Captain will be sure to inspect all doors once a week for compliance. 2. Staff will be retrained on or before December 9, 2016 on sensory material safety and age appropriate activities. Management will observe daily for compliance.
Standard #: 22VAC40-185-450-A Description: Based on observation the center failed to ensure cots used by children consisted of a top and bottom cover. Evidence: In the 2 year-old room 3 of the 10 children observed napping on cots did not have a top cover. Plan of Correction: All parents were notified the same day of the inspection to please provide blankets. The staff were retrained on 12/ 8/16 where additional sheets and blankets are stored should parents not provide one. Management will observe weekly for compliance.
Standard #: 22VAC40-185-560-F Description: Based on observation and a record review the center failed to ensure all requirements were met when providing food. Evidence: The menu posted in the front lobby read that the children were eating Cheese Quesadillas for lunch. The children were observed eating chicken and gravy during lunch time. Plan of Correction: Food Prep Specialist was retrained on changing menu prior to food presentation daily on Dec. 6, 2016. Management will check daily for compliance.
Standard #: 22VAC40-185-560-M Description: Based on observation the center failed to ensure staff sit with the children during meals time. Evidence: In the preschool room children were observed eating lunch. The staff in the room was observed cleaning up and was not sitting with the children during their meal. Plan of Correction: All staff have been retrained as of Dec 9, 2016 on meal time policies. Management will observe daily for compliance
