An unannounced renewal inspection was conducted today from 7:35 - 8:55 A.M. There were 15 children with 4 staff when the inspector arrived and 50 children with 5 staff when the inspector left. The children played inside and outside and were observed reading, coloring , making stained glass windows, painting "bat" pictures, making bracelets and playing with trains. The exit interview was conducted with the head teacher as the site supervisor was absent. The violation notice will be e mailed for completion.

Description: Based on a review of children's records, the licensee did not obtain an immunization record for 1 child. Evidence - The record for child #6 did not contain an immunization record.

Plan of Correction: Immunization filed in child #6 folder.

Standard #: 22VAC40-185-140-A
Description: Based on a review of children's records, the licensee did not obtain a physical examination for 1 child. Evidence - The record for child #6 did not contain a physical examination.

Standard #: 22VAC40-185-60-A
Description: Based on a review of children's files, the licensee did not obtain all of the required information for 3 children. Evidence - The records for children #6 & #8 had no action to take if the child is exposed to a known allergen. The record for child #9 had the parents listed as the child's emergency contacts.

Plan of Correction: Parents wrote in action to be taken if child is exposed to allergen. Parent provided new emergency contact

Standard #: 22VAC40-185-260-A
Description: Based on a review of the most recent fire inspection and fire permit, the licensee did not able to provide an annual fire inspection report. Evidence - The most recent fire permit expired on 9/30/2016.

Plan of Correction: - schools main office will provide - The office called fire inspection to make appointment

Standard #: 22VAC40-185-540-E
Description: Based on observation, the licensee did not have a working battery operated radio. Evidence - The available radio did not work as it did not contain batteries.

Plan of Correction: Batteries were installed in the radio.
