Comments:
An unannounced monitoring inspection was conducted today. Observed 35 children directly supervised by 5 staff members. Ratios and supervision were in compliance. A sample of 2 children's records, and 5 staff emergency contact information sheets were reviewed. The physical plant, medications, emergency supplies and procedures were also reviewed. The children were engaged in various activities, to include: free play, snack time, and hand washing. Areas of non-compliance are identified in this report. The exit interview was conducted with the Site Director. Inspection conducted 2:35pm-3:15pm. Description: Based upon observations made and documentation reviewed, a child's record was not kept confidentially. Evidence: Confidential information was posted and readily viewable by the public and parents. Identifying information about a child referenced on the Supplemental Information portion of the November 17, 2015 inspection report was posted. The report states, "Information found on the Supplemental Information page is confidential and this document is not to be posted in the facility".

Description: Based upon documentation reviewed, the center did not provide to the licensing representative an annual fire inspection report from the appropriate fire official having jurisdiction. Evidence: The last fire inspection document posted on the parent's board is dated 09/01/2016.
