Comments:
Today's visit was a routine monitoring inspection. The inspection began at 8:30am and concluded at 12:30pm. There were forty children present with six staff and additional staff were present in the building. During the inspection the school age space was viewed and measured, the children's classrooms were observed and holiday parties were taking place, five out of one hundred ninety children's records were reviewed, emergency supplies were viewed, a sample of medications were viewed along with authorization, five out of twenty-four staff records were reviewed, and some policies and procedures were reviewed. Discussion took place on ways in which Program Leader Qualifications are documented. Thank you for your time, please call me at 309-2051 if you have any questions.

Violations:
Standard #: 22VAC40-185-40-E
Description: Based on the inspector's review of the license and the programs that the center offers, the center failed to ensure they were with the age group indicated on the current license. Evidence: The inspector interviewed the the Program Director to review what programs and age groups are currently being served at the center. Based on the interview the center is currently serving children above the age of nine and half that is currently indicated on the license. An after school program has been offered for elementary school aged children as of September of this year.

Plan of Correction: A request to modify the age group and add additional capacity was submitted to the licensing inspector during the time of the visit.

Standard #: 22VAC40-185-550-C
Description: Based on the inspector's observations, procedures for emergency evacuation and sheltering in place were not posted on each floor of each building. Evidence: Emergency procedures to include evacuation and sheltering in sheltering in place were not posted on each floor of the building that is used by children in care.

Plan of Correction: The procedures will be posted on each floor used by children in care by the end of the program day.

Standard #: 22VAC40-191-60-B
Description: Based on the inspector's review of five out of twenty-four staff records, the center failed to ensure the record for a staff member had signed sworn statement prior to first day of employment. Evidence: The record for staff member #5 has a documented date of hire as 9/17/15 and the sign sworn statement is date 9/8/16. An employee is required to complete a signed sworn statement before employment.

Plan of Correction: The PD will ensure all staff complete a signed sworn statement prior to employment.

Standard #: 22VAC40-191-60-C-1
Description: Based on the inspector's review of five out of twenty-four children's records, the center failed to ensure the results of the criminal background check was received within thirty days from date of employment. Evidence: The record for staff member #5 has a document hire date as of 9/17/15 and the criminal background check is signed and dated complete by the agency responsible for completing the background check as of 10/19/16. Background checks are required to be completed within thirty days of employment.

Plan of Correction: The PD will ensure the background check requests are submitted within one week of date of hire and completed within thirty days upon date of hire. Follow-up will take place and be documented if not received within thirty days.

Standard #: 22VAC40-191-60-C-2
Description: Based on the inspector's review of five out of twenty-four children's records, the center failed to ensure the results of the search of the central registry background check was received within thirty days from date of employment. Evidence: The record for staff member #5 has a document hire date as of 9/17/15 and the search of the central registry is signed and dated complete by the agency responsible for completing the background check as of 9/29/16. Background checks are required to be completed within thirty days of employment.

Plan of Correction: The PD will ensure the background check requests are submitted within one week of date of hire and completed within thirty days upon date of hire. Follow-up will take place and be documented if not received within thirty days.
