Thank you for your time and assistance with the unannounced monitoring inspection conducted today, December 15, 2016. The on-site inspection was from 10:25 a.m. to 3:40 p.m. On this date, at the time of arrival, there were 73 children in the care of 13 staff members. Two extra staff members were available to assist, if needed. There were 125 children enrolled in the program. There were 12 new staff members' records reviewed and the records of two staff members were updated. Five children?s records were reviewed. The medication and medication paperwork were reviewed for six children. There were eight staff members and three children interviewed, and six classrooms observed. The exception to compliance was noted on the violation notice and the risk rating associated with the violation was noted on the supplemental page. Lunch was observed to meet USDA guidelines. An inter-generational Christmas activity, Christmas art projects, center time, circle time, story time, outdoor play, infant care, diaper changing procedures, hand washing and bathroom procedures, lunch, and nap were observed during the visit. Standard #: 22VAC40-185-90--A Description: Based on review of five children's records and verification from the director, the center failed to obtain the required written agreements between the parents and the center for one child by the child's first day of attendance. Evidence: 1. The record of Child #1 contained the parent/center written agreements that were signed and dated by the parent on 12/01/2016. The child's first day of attendance was 11/28/2016. 2. The director confirmed the agreements were not signed by the parent by the child's first day of attendance. Plan of Correction: The director is now aware that the agreements must be signed by the parent before the child can attend. She will ensure all parent/center agreements are completed prior to a child's first day of attendance.
