Please remember that prior to renewal all members of the board need to have current background checks on file available for review.
Licensing Inspector arrived at 9:15 and departed at 10:30 PM. At the time of the inspection there were 23 children ages two and a half through five in care with five staff. Children were observed participating in free choice activities. The sample size was three staff and three children's records. A violation was noted in the area of administration.
Standard #: 22VAC40-185-70-A Description: Based on a review of four staff records, it was determined that the facility did not ensure that all required elements were present in each staff member's record. Evidence: 1. The record for staff #1 did not include a employment start date or documentation of the two required references as to character & reputation. 2. The record for staff #2 did not include documentation of orientation training. 3. The record for staff #3 did not include documentation of two required references as to character & reputation and documentation of orientation training. Plan of Correction: Facility management will ensure that all required elements are present in each staff member's record.
