An unannounced monitoring inspection was conducted beginning at 4:15 p.m. and ending at 5:10 p.m. with the director and one staff. There were fifteen children in care ranging in age from five years to twelve years of age. Children were observed finishing snack and preparing for bathroom time. Snack today was "Fun Friday Snack" and staff made No Bake Oatmeal Bars. After bathroom time, children played at centers that included a new board game called 'googly eyes' or ping pong. Two medications and authorizations were reviewed. Two staff records and four children's records were reviewed. Standard #: 22VAC40-185-60-A Description: Based on review of documentation, not all children's records were complete. Evidence: Child #4, enrolled 8/16/2016, did not have a full address for a second emergency contact. Plan of Correction: The information will be requested from the parents again.
