An unannounced monitoring inspection was conducted today 12/21/2016. The inspection began at 8:40 A.M. and concluded at 12:45pm. There were 75 children present with 13 staff members and the Program Director's were also present during the inspection. During the the time of the inspection all eight classrooms were observed, infant care was observed, diapering was observed, hand washing was observed,morning activities, arrival into the program, breakfast, outdoor time, lunch time was observed, emergency supplies were viewed, one medications were and authorizations for the medications was reviewed. Discussion with the program director about: main office documenting verification of background checks, groupings of children, and lead teacher requirements.
Standard #: 22VAC40-185-70-A Description: Based on review of five staff's records, the center failed to ensure that all records were complete. Evidence: 1. The inspector found that staff # 4 did not have a documented job title. Plan of Correction: During the inspection the director documented the job title.
Standard #: 22VAC40-185-270-A Description: Based on observation of the three year old preschool classroom, the center failed to ensure that all areas and equipment inside and outside was maintained in a clean, safe and operable condition. Evidence: 1. The inspector observed in the preschool classroom a drawer that was missing the front panel and had exposed screws that protruded out and posed a hazard to the children in care. The drawer was accessible to the children in care, next to the hand washing sink used by the children. Plan of Correction: The PD will contact the Maintenance Department today and submit a work order to have the drawer repaired as soon as possible. Staff will be reminded to report safety hazards to the PD immediately. 12/22
Standard #: 22VAC40-185-280-G Description: Based on observation of classrooms, the center failed to ensure that all substitute container clearly indicate their contents. Evidence: 1. The inspector observed two substitute containers in the school age room that did not clearly indicate their contents. 2. In the head start 2 room there was one substitute container that did not clearly indicate their contents . 3. In the three year old preschool class room there was one container that did not clearly indicate their contents. Plan of Correction: Staff will be reminded to properly label all containers in the classroom. The PD will ensure that all containers are labeled by checking them weekly. 12/21
Standard #: 22VAC40-185-340-D Description: Based on observation and record review, the center failed to ensure that all grouping of children had a qualified program lead teacher. Evidence: 1. The inspector observed staff # 5 working with a grouping of six two year old children. The room has a barrier that was opened during this time that allowed for the program leader that was with separate grouping of children to look in on the classroom. Staff #5 did not have the qualification for program lead. The requirement is for each grouping of children to have at least one staff member present that meets the qualifications of program leader.
