An unannounced monitoring inspection was completed on December 27, 2016. The purpose of this visit was to measure the facility?s compliance with most but not all of, the licensing standards. The following ratios were observed: Infants-8 children/2 staff Toddlers/Discovery preschool- 10 children/2 staff Preschool - 10 children/2 staff School age- 13 children/1 staff Due to the holiday, many children were not in attendance so a couple of the classrooms were combined. The infants were observed napping, being held and playing with toys on the floor. The toddlers were playing with toys on the floor with staff and were being diapered. The preschoolers were observed playing on the playground and afterward, they were engaged in story time. The school aged children were engaged in free choice activities including board games, tinker toys and other activities. The children were served a lunch of chicken spaghetti, mashed potatoes, mandarin oranges and milk. The facility administers medication. Medication and accompanying authorizations were reviewed. The last evacuation drill was conducted 10/28/16. The most recent shelter in place drill was conducted 2/25/16. Four staff records and four children?s records were reviewed. Standard #: 22VAC40-185-160-A Description: Based on a review of a sample of staff records, a new staff person did not have tuberculosis screening results within 21 days of hire. EVIDENCE: The record of staff person # 4, start date 11/7/16, did not contain results from a tuberculosis screening. Plan of Correction: Staff will get tuberculosis screening results within the week.
Standard #: 22VAC40-185-550-D Description: Based on a review of the Evacuation and Disaster Drill Log, the facility did not ensure that monthly evacuation drills were conducted. EVIDENCE: The evacuation drills were not conducted monthly. There was no drill recorded for the month of August, and the last drill recorded was conducted 10/28/16, 60 days prior to today. Plan of Correction: The training for our new health and safety person will be finished. Evacuation drills will be conducted monthly.
Standard #: 22VAC40-191-60-C-1 Description: Based on a review of a sample of staff person's records, the facility did not ensure that all staff have an original criminal history record report within 30 days of employment. EVIDENCE: The record of staff person #4, hire date 11/7/16, did not contain an original criminal history record report. The criminal record request was originally submitted 11/7/16, but was returned to the facility on 12/1/16 for correction. The request was not resubmitted until 12/21/16. Plan of Correction: We will contact the agency to see if the request was received and will resubmit if required. The staff person will be removed from childcare duties until the background clearance is received.
Standard #: 22VAC40-191-60-C-2 Description: Based on a review of staff records, the facility did not ensure that all staff had a central registry finding within 30 days of employment. EVIDENCE: 1) The record of staff person #3, start date 10/17/16, did not contain central registry findings, nor did it contain documentation of submission and inquiry concerning the clearance. Plan of Correction: We will contact the agency to see if the request was received and will resubmit if required. The staff person will be removed from childcare duties until the background clearance is received.
