The licensing inspector conducted an unannounced renewal inspection Thursday, December 15, 2016 from1:20pm to2:30pm. The children were observed resting. The center was preparing for their annual Christmas program that afternoon. All classrooms were inspected today. The center does not provide transportation. Three children?s records and one employee records were reviewed during this inspection. Medication is not being administered. Ten injury reports were reviewed. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 11/30/2016 Shelter-in-place drills: 03/22/2016 & 10/03/2016 Last fire inspection: 12/05/2016 Last health inspection: 10/06/2016 Total enrollment today: 21 Today, the following child to staff rest time ratios were observed: Twos ? 10:1 Threes ?4:1 Pre-K ?7:1 The violations from the previous inspections were checked for corrections. Standard #: 22VAC40-185-140-A Description: Based on a review of children's records on 12/15/2016, the center did not ensure that one of the three records reviewed contained a physical examination within one month after attendance. EVIDENCE: The physical examination in the record of Child # 2, whose first day of attendance at the center was 09/02/2014, is dated 04/10/2015. Plan of Correction: Going forward will make sure forms are in within 30 days.
Standard #: 22VAC40-185-550-M Description: Based on a review of documentation on 12/15/2016, the center did not ensure that the record of injuries contained the date and time parents were notified. EVIDENCE: 1. Out of a sample of 10, six injury reports did not contain the time the parent was notified. 2. Out of a sample of 10, three injury reports did not contain the date the parent was notified. Plan of Correction: Will make sure the date and time of communication is filled in on injury reports.
Standard #: 22VAC40-191-40-D-1-A Description: Based on staff interview on 12/15/2016, the center did not ensure that any new person designated as licensee has a sworn statement or affirmation, search of central registry and criminal history record check before the end of 30 days after the change. EVIDENCE: Staff # 2 stated that the record for Board Member #2, who was listed on the renewal application received by the department on 11/07/2016, did not have a sworn statement or affirmation, search of central registry and criminal history record check. Plan of Correction: Will get sent out by 12/28/2016.

EVIDENCE: 1. The most recent sworn statement for Board Member #1is dated 09/14/2012. 2. The most recent central registry search for Board Member #1is dated 11/06/2012. 3. The most recent criminal history record check for Board Member #1is dated 10/09/2012. Plan of Correction: Will get sent out by 12/28/2016.
