The licensing inspector conducted an unannounced monitoring inspection Friday, December 16, 2016 from 10:01am to 12:30pm. The children were observed engaged in a variety of activities ranging from drawing to playing with toys. The infants were observed playing on the floor and being fed. Lunch consisted of meatloaf, mashed potatoes, mandarin oranges, rolls and milk. The lunch menu was posted. All classrooms and playgrounds were inspected today as well as the facility?s mini buses. Four children?s records and two employee records were reviewed during this inspection. Medication is being administered and was reviewed. Ten injury reports were reviewed as well as the center?s injury prevention plan which was updated September 2016. The center's first aid kit and emergency supplies were inspected. Last emergency drill:11/28/2016 Shelter-in-place drills: 2/23/2016 & 10/18/2016 Last fire inspection: 09/12/2016 Last health inspection: 12/02/2016 Total enrollment today:83 Today, the following child to staff ratios were observed: Infants ?7:3 Pre-Toddlers ?7:2 Toddlers ?10:2 Twos ? 22:3 Threes ?16:2 Pre-K ?21:3 School Age ?not present The violations from the previous inspections were checked for corrections. Standard #: 22VAC40-185-270-A Description: Based on a observation on 12/16/2016 the center did not ensure that all areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Unsafe conditions shall include, but not be limited to, splintered, cracked or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting or breakage of any equipment. EVIDENCE: Two floor boards on the platform of the slide equipment on the toddler playground are broken. One is chipped and missing the screw which can cause a pinching hazard. The other is cracked at the screw, which can also cause a pinching hazard. Plan of Correction: Emailed the executive director about the broken equipment today, 12/16/2016. Will not have children play on the equipment until repaired.
Standard #: 22VAC40-185-280-B Description: Based on observation on 12/16/2016, the center did not ensure that all hazardous substances such as cleaning materials, insecticides, were kept in a locked place that prevents access by children. EVIDENCE: 1. A canister of spray cleaner, and disinfectant spray were observed in the toddler room on top of the cabinet. The items were out of the reach of the children. 2. In the Pre-Toddler class a canister of spray cleaner, and disinfectant spray were observed under the diaper changing table. 3. On Bus #1 a can of disinfectant spray, a can of WD-40 and other cleaning products were observed in a basket on the floor in the front of the bus. Plan of Correction: Corrected on site, items were removed or placed in a locked cabinet.
Standard #: 22VAC40-185-550-M Description: Based on a review of documentation on 12/16/2016 the center did not ensure that all injury reports contained the date and time parents were notified; EVIDENCE: 1. Out of a sample of 10 injury reports, seven did not have the time when the parent was notified. 2. Out of a sample of 10 injury reports, five did not have the date when the parent was notified. Plan of Correction: A memo will be sent out today to remind teachers to complete the required areas of the injury report form.
