Technical Assistance:

The licensing inspector conducted an unannounced renewal inspection Thursday, December 22, 2016 from 11:40am to 1:50pm. The toddlers and preschool children were observed resting. The two's were observed getting ready for rest time and the school age children were observed getting ready to leave the center to go practice for the Christmas play that afternoon. The infants were observed playing with toys on the floor and sleeping on cots as well. Lunch consisted of chicken chilli with pasta, green beans, pineapple tidbits and milk. All classrooms and playgrounds were inspected today as well as the facility?s mini buses. Three children?s records and four employee records were reviewed during this inspection. The center administers medication; however they did not have any being given at this time. Three injury reports were reviewed. The center's first aid kit and emergency supplies were inspected. Last emergency drill:12/13/2016 Last shelter-in-place drill:1/19/2016 Last fire inspection: 10/18/2016 Last health inspection: 11/18/2016 Total enrollment today: 63 Today, the following child to staff ratios were observed: Infants ?6:2 Toddlers ?5:1 Twos ? 15:2 Threes ?13:2 School Age ?24:2 The violations from the previous inspections were checked for corrections. There were no repeat violations found during today?s inspection. The Department has not yet received a complete renewal application and annual fee from the center. The Department must receive a complete renewal application and fee prior to expiration of the license on 02/14/2016 or the Department cannot renew the license. Description: Based on observation on 12/22/1016 the center did not ensure that hazardous substances such as cleaning materials, were kept in a locked place using a safe locking method that prevents access by children. EVIDENCE: Two spray bottles labeled " bleach water" were observed in a cabinet under the sink in the school age classroom.

Plan of Correction: Corrected on site, spray bottles moved to a locked cabinet.

Description: Based on a review of children's records on 12/22/2106, the center did not ensure that three of three records reviewed contained documentation of the parent confirmation that the required information in the child's record is up to date annually. EVIDENCE: 1. The most recent update of information documented in the record of Child #1 is dated 11/17/2014. 2. The most recent update of information documented in the record of Child #2 is dated 04/14/2015. 3. The most recent update of information documented in the record of Child #3 is dated 08/03/2015.

Plan of Correction: Updates were completed, however need to locate the documentation. Proof of update will be in files by 01/15/2017.
