22VAC40-191 Background Checks (22VAC40-191)
An unannounced Renewal inspection was conducted in the Center on December 19, 2016. The LI arrived in the Center at 11:00AM. There were 46 children present, the children were grouped in 4 classrooms. Eleven staff members were assigned to the classrooms and 3 additional administrative staff were also present during the inspection. The children's ages ranged from infancy to pre-school age. As reported, 56 children are currently enrolled in the program. A sample of 4 children's records was reviewed. Two partial staff records were reviewed. Complete staff records are maintained at the central administration office, a sample of 4 staff records was reviewed on December 21, 2016. The children were observed in each classroom as they participated in varied activities which included music and free play. Upon entrance to one of the classrooms, staff was observed reading a book to the children. The lunch meal was also observed. After lunch the children had scheduled nap/rest time. Dismissal was observed at the end of the program day. Documentation of the Center's annual fire and health inspections was current. Prescribed medications and written authorizations to administer were reviewed for two children. The LI observed hand washing, feeding and diapering procedures. Staff were engaged with the children and were attentive to their needs. The outdoor playgrounds were inspected. Findings of the inspection were reviewed with the Site Supervisor. Standard #: 22VAC40-185-60-A Description: Based on a sample review of children's records, the center failed to obtain documentation to verify legal custody of a child in care. Evidence: A sample of four children's records was reviewed. One of the four children was enrolled in the Center by someone other than a natural parent, documentation of a legal custody agreement was not maintained in the record (child 3). Plan of Correction: The Family Development Specialist will contact the individual to obtain documentation to verify legal custody, documentation will be maintained in the child's record.
Standard #: 22VAC40-185-510-A Description: Based on reviewed documentation, the Center failed to obtain written authorization from a parent to give prescription and nonprescription medication. Evidence: The LI observed a prescribed medication in the center for child a. Several different documents contained the physician's signature for long-term administration, however the parent's signature was not on the authorization forms. Plan of Correction: Staff will notify the parent and will obtain signed authorization for long-term administration.
Standard #: 22VAC40-185-550-M Description: Based on reviewed documentation, the Center failed to maintain a complete written record of children's minor injuries to include: the time when parents were notified; time of injury. The center failed to notify a parent by the end of the day of any known minor injuries. Evidence: 1. The LI reviewed 3 written minor injury records. The record for child b. (9/20/16) noted the child fell backwards and received a knot on head, the record did not indicate the time the parent was informed of the injury; 2. A recorded incident on 08/18 in the Toddler classroom for a child whom was bitten by another child did not indicate the time of the injury. 3. Minor injury records for child c. dated 10/17/16 and 11/17/16 noted the child had received minor injuries, documentation indicated that the parent was informed the following day on both occurrences. Plan of Correction: The Supervisor will review completed injury records with staff to ensure all required information is completed on each record. Records will be reviewed on-going.
