Technical Assistance:
**Discussed with staff that they needed to ensure that hand towels and hand soap are accessible to children. **Discussed with staff that reference checks need to be completed prior to employment and not on or after their first day of work.

An unannounced monitoring inspection was conducted today at 9:20am. There were 20 (infants-school-agers) directly supervised by 5 staff. The physical plant, 3 staff records, 4 children?s records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, circle time and playing outside. There was an abundant supply of books, toys, and materials for the children. The center was clean and organized. Areas of non-compliance are identified in this report. Please sign and date all 3 reports and return it to me within 10 calendar days from today. Description: Based on review, the facility failed to ensure that staff records were maintained with the required documentation. Evidence: Reviewed 3 staff records and found that Staff 1 had documentation of references completed on the day that the staff member started work. Staff member 1 started work in the morning, and reference were not completed prior to employment.

Plan of Correction: References will be done before staff is hired.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the facility failed to ensure that the areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Evidence: Observed a tear in the top of the "cow" print reading pillow accessible to children in the Hershey's Kisses Room.

Plan of Correction: It will be fixed.

Standard #: 22VAC40-185-430-B
Description: Based on observation, the facility failed to ensure that materials and equipment available are age and stage appropriate for the children. Evidence: Observed that the art projects completed by the children were stored on a wire shelf and contained small chokable plastic tubes that could become a potential choking hazard.

Plan of Correction: That will be sent home today.

Standard #: 22VAC40-185-550-M
Description: Based on review, the facility failed to ensure that injury/accident reports are maintained with the required documentation. Evidence: Reviewed 4 injury/accident reports and found that 1 was missing documentation of the time parents were notified of the incident and 1 was missing documentation of any future action to prevent recurrence.

Plan of Correction: Remind teachers to put the date and the future plan of action.
