Comments:
An unannounced monitoring inspection was conducted today. Observed 10 children directly supervised by 4 staff members. Ratios and supervision were in compliance. A sample of 2 children's records, and 2 staff records were reviewed. The physical plant, medications, emergency supplies and procedures were also reviewed. The children were engaged in various activities, to include: lunch, free play, and infant care. Areas of non-compliance are identified in this report. Standard #: 22VAC40-185-130-A
Description: Based upon documentation reviewed and observations, the center did not obtain documentation that a child has received immunizations as required before the child can attend the center. Evidence: One of the two children's records reviewed did not contain documentation of the child's immunizations record (Child B; DOE 09/2016). Child B was present during today's inspection.

Description: Based upon documentation reviewed, a child's physical examination was not obtained prior to attendance or within a month after attendance. Evidence: One of the two children's records reviewed did not contain a physical examination (Child B; DOE 09/2016).

Description: Based upon documentation reviewed, children's records were incomplete. Evidence: 1) Two of the two children's records reviewed did not contain the name, address, and telephone number for a second emergency contact (Child A; Date of enrollment (DOE) 11/21/2016, and Child B; DOE 09/2016). 2) One of the two children's records reviewed did not contain documentation verifying the child's age and identity (Child B; DOE 09/2016).

Description: Based upon documentation reviewed, and interview, there was not at least one staff member on duty who has obtained within the last three years instruction in performing the daily health observation (DHO) of children. Evidence: During today's inspection, none of the staff present had obtained the DHO training. There are only 2 staff members who currently have the training and were not present today.

Description: Based upon observations made, an equipment of the center was not kept in safe condition. Evidence: The big rectangular carpet in the three years old classroom was ripped on one of the edges, creating a possible tripping hazard.

Description: Based upon observations made, hazardous substances were not kept in a locked place using a safe locking method that prevents access by children. Evidence: 1) Disinfectant wipes were observed on top of the pretend kitchen in the three year old classroom. 2) A total of five disinfectant wipes were observed on top a shelf in the four year old classroom. 3) A plastic container of air bubbles air freshener was observed on top of a shelf in the two year old classroom.

Description: Based upon observations made, electrical outlets did not have protective covers. Evidence: 1) An electrical outlet in the three year old classroom by the circle time poster did not have one of the two required protective covers. 2) An electrical outlet in the three year old classroom by the art center did not have one of the two required protective covers. 3) An electrical outlet in the two year old classroom was missing two of the two required protective covers.
