Comments:
An unannounced monitoring inspection was conducted on December 20th 2016 between 11:30am and 2:30pm. There were 61 children in the direct care of 16 staff members. Children were observed finishing their lunch, washing their hands, using the rest room, preparing their cots, reading and napping. Staff were consistently engaged with the children using positive redirection to help transition between activities. The center is organized, clean, and offers plenty of materials for children to utilize. A sampling of children's records, staffing records, the fire inspection, evacuation drills, physical space, and supervision were observed and inspected. Standard #: 22VAC40-185-60-A
Description: Based on record review and interview, 2 children in care do not have records on file. Evidence: The director told the inspector that 2 children in care are children of an employee and they do not have records at the facility.

Plan of Correction: We will get files completed on any children of staff who are here on some occasions.

Standard #: 22VAC40-185-280-B
Description: Based on observation, all cleaning materials were not kept in a locked place using a safe locking method. Evidence: In room 119, cleaning sprays were in a cabinet that had a broken lock.

Plan of Correction: Lock will be repaired. Cleaning materials were moved to a locked cabinet during inspection.

Standard #: 22VAC40-191-40-D-1-B
Description: Based on record review, 2 of 4 staffing records reviewed did not have all the required background checks completed within 30 days of employment at the facility. Evidence: Staff #1 and Staff #4 both started employment October 2016. There is no documentation of a criminal record check in the staff file and the central registry search was mailed on November 18th 2016 but has yet to be returned from the agency.

Plan of Correction: Director will contact VA DSS to check on record checks.
