Discussion on topics to include: Posted inspection findings should include all pages of documents except the SUPPLEMENTAL INFORMATION page. Model topical ointment form should not be considered a blanket form for all topical cream applications.

Comments:
An unannounced renewal inspection was conducted today, January 5, 2017 from 11:25am until 1:00pm . Observed 17 children directly supervised by 3 staff members. Ratios and supervision were maintained at a 1:7 ratio per company policy which exceeds the supervision requirement set forth in the minimum requirements. A sample of 4 children's records and 2 staff records were reviewed. The physical plant, emergency supplies and procedures were also reviewed. No medication The children were engaged in various activities, to include: playing with manipulative, art activities and lunch consisting of turkey hot dogs, oranges and salad. Areas of non-compliance are identified in this report. The exit interview was conducted with the Director. Description: Based on record review, one of two staff records checked was incomplete. Evidence: Staff#2's record did not contain 2 required reference checks.

Plan of Correction: We will obtain the reference checks.

Standard #: 22VAC40-185-550-M
Description: Based on record review, the center's injury reports does not include all required information. Evidence: The report does not include a space to record a method to prevent reoccurrence.

Plan of Correction: The center will re create this form to include all required information.
