An unannounced investigation that was conducted on December 30, 2016 and January 3, 2017. The on-site portion of the inspection was held on January 3, 2017 from 10:40 a.m. to 12:00 p.m. It was in regards to a self-reported incident which was received in the Valley Licensing Office on December 28, 2016. Three staff members were interviewed. There was one child's record and two staff members' records reviewed. The violation resulting from the inspection was listed on the violation notice. There were 80 children present on January 3, 2017, and 126 children enrolled in the program. An exit interview was conducted on January 3, 2017. Description: Based on staff interviews and a written statement by staff, it was determined that a staff member closed a child in a space that the child could not freely exit. Evidence: 1. In Classroom A, Staff Member #1 closed the bathroom door with Child A inside and held the doorknob to keep the door closed for an undetermined amount of time but no longer than a minute. 2. Staff Member #1 reported that Child A was in the bathroom. She closed the door with the child inside and held the doorknob. 3. Staff Member #2 reported that she saw Staff Member #1 shut the bathroom door with Child A inside and hold the door shut.

Plan of Correction: Staff Member #1 was sent home that day and terminated as an employee. We have added to our existing new hire forms for discipline policy a list of forbidden actions. These will be discussed at length with all new staff. We will have a meeting and discuss forbidden actions with our current staff to reinforce severity.
