Technical Assistance:

The licensing inspector conducted an unannounced renewal inspection Friday, January 6, 2017 from approximately 9:50am to 1:40pm. The following ratios were observed with a total of 44 children present during the inspection: Infants - 7 children with 2 staff Toddlers - 10 children with 2 staff Discovery Preschool (Twos)- 10 children with 2 staff Threes- 10 children with 1 staff Pre-K - 7 children with 1 staff School Age children were not present during the inspection. The children were observed engaged in a variety of activities during the inspection. The Infants were observed sleeping in their cribs, being held and rocked, playing on the floor with toys and eating lunch. The Toddlers were observed eating lunch and later resting at nap time. The Twos were observed learning about weather and creating lightning bolts during art. The Threes were observed playing in centers including home living, blocks and library. Later they were observed playing with bean bags while dancing to music. The Pre-K children were observed participating in center time where the children played with cars on the road rug, looked at books, played transportation bingo, played with trains in the sensory table, played computer games and practiced their writing skills. Later children were observed eating lunch. Lunch consisted of cheese pizza, green beans, fruit and milk. All areas of the facility including classrooms, hallways, playground and bathrooms were inspected. The facility was equipped with toys and supplies and items were available to the children. The following information was reviewed: injury reports, daily attendance, emergency preparedness plan, practiced emergency drill logs, staff training information and liability insurance. Medication is administered and medication and medication authorizations were inspected. Transportation is provided and vehicles and transportation documents were reviewed. Last documented health inspection: 01/26/16 Last documented fire inspection: 02/26/16 During the inspection, five children's records and five staff records were reviewed. The risk profile tool will be used for the licensing recommendation once all of the necessary paperwork is received by the licensing department. The renewal application was received prior to the expiration of the current license. Until the new license is received, the provider is legally operating. Description: Based on observation, the facility did not maintain all areas in a safe and operable condition. Evidence: During the inspection, the licensing inspector observed an exposed rusting bolt at the base of the toilet located in the Preschool classroom bathroom and School Age classroom bathroom.

Plan of Correction: The maintenance person was on site during the inspection stated he would place a cover over the exposed bolts.
