An unannounced on-site investigation was conducted on January 6, 2017 from 3:30a.m. to 4:50p.m. in response to a self-reported concern which was received in the Valley Licensing Office on January 4, 2017. Appropriate staff members were interviewed and two children's records were reviewed and one staff member's record was checked for updates. Classroom furniture arrangements were observed and discussed with staff. The violation resulting from the inspection was listed on the violation notice. There were 80 children present at the time of the inspection, and 144 children were enrolled in the program. An exit interview was conducted on January 6, 2017. Description: Based on observation reports and interviews with staff, the center failed to provide sight supervision of children at all times. Evidence: 1. On 01/04/2017, at approximately 4:00 p.m., Child A (age four years) and Child B (age three years) were not in direct sight supervision of staff. The arrangement of furniture in Classroom A created a blind spot in the Computer Center, and supervising Staff Member #1 was unable to see interactions between the two children.

Plan of Correction: With regards to the aforementioned violation, Children of America will be more diligent and meticulous with evaluating the classroom layout at the beginning of each day prior to the children's arrival. From this evaluation, center management and classroom teachers will collaborate to determine the most appropriate room arrangement for the day. Teachers will continue to do their periodic sweeps of the classroom and conscientious monitoring while children are in their care.
