Comments:
The licensing inspector conducted an unannounced monitoring inspection Thursday, January 5, 2017 from 10:15am to 11:40am. The children were observed engaged in singing songs, dancing, and learning about the letter Mm. The infants were observed drinking a bottle, getting ready for a nap and playing with their teacher on the floor. The lunch menu was posted and today lunch was to consist of a peanut butter an jelly sandwich on wheat bread, carrots, cucumbers, and milk. All classrooms and the playground were inspected today. The facility does not provide transportation. Three children?s records and three employee records were reviewed during this inspection. The center administers medication; however they did not have any being given at this time. The center's first aid kit and emergency supplies were inspected. Last emergency drill:12/22/2016 Shelter-in-place drills: 03/28/2016 & 11/22/2016 Last fire inspection: 12/28/2016 Last health inspection: 12/19/2016 Total enrollment today: 19 Today, the following child to staff ratios were observed: Infants ?6:2 Toddlers ?5:1 Preschool ?8:1 The violations from the previous inspections were checked for corrections. Violations:
Standard #: 22VAC40-185-520-A
Description: Based on observation on 01/05/2017, the center did not ensure that over-thecounter skin products shall not be kept or used beyond the expiration date of the product. EVIDENCE: A tube of petroleum jelly belonging to Child #4 was observed in the toddler room. The petroleum jelly expired 05/2015.

Plan of Correction: Will be given back to parent today. In the future will implement a policy to check the dates more frequently.

Standard #: 22VAC40-191-60-C-1
Description: Based on a review of staff records on 01/05/2017, the center did not ensure that an employee is denied continued employment if they do not have an original criminal history record report within 30 days of employment. EVIDENCE: 1. The record of Staff #1, whose date of hire was 08/28/2016 and was working at the center on 01/05/2017, did not contain documentation of an original criminal history record report. 2. The record of Staff #2, whose date of hire was 09/06/2016 and was working at the center on 01/05/2017, did not contain documentation of an original criminal history record report. 3. The record of Staff #3, whose date of hire was 11/28/2016 and was working at the center on 01/05/2017, did not contain documentation of an original criminal history record report.

Plan of Correction: Sent off forms after hire, however when they didn't come back, the center contacted the agencies and was informed that the forms were not received. They were sent again mid December 2016. Will check to follow up if not received by Monday, 1/09/2017.
