The licensing inspector conducted an unannounced monitoring inspection Friday, January 6, 2017 from12:55pm to 3:20pm. The children were observed transitioning to the afternoon enrichment program. In afternoon enrichment the children were observed playing in the gym. The two Junior Kindergarten classes were observed playing games like "Penguins on Ice" and painting fish. Interviews were held with staff throughout the inspection, and the inspector interacted with children in each classroom when appropriate. All classrooms and playgrounds were inspected today. Three children?s records and ten new employee records were reviewed during this inspection. Medication is being administered and was reviewed. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 12/13/2016 Shelter-in-place drills: 05/03/2016 & 10/20/2016 Today, the following child to staff ratios were observed: Afternoon Enrichment 1(three to five years old)-8:2 Afternoon Enrichment 2 (three to five years old)-7:1 Junior K, Green Room (four to five years old)-8:2 Junior K, Rainbow Room (four to five years old)-8:2 The violations from the previous inspections were checked for corrections. Standard #: 22VAC40-185-160-A Description: Based on a review of staff records on 01/06/2017, the center did not ensure that three of eight records reviewed contained documentation of a negative tuberculosis (tb) screening no later than 21 days after employment. EVIDENCE: 1. The earliest documentation of a negative tb screening in the record of Staff #1, who started at the center on 09/06/2016, is dated 12/07/2016. 2. The earliest documentation of a negative tb screening in the record of Staff #5, who started at the center on 09/06/2016, is dated 12/07/2016. 3. The earliest documentation of a negative tb screening in the record of Staff #6, who started at the center on 09/06/2016, is dated 12/09/2016. Plan of Correction: If staff miss the center tb test day they will be sent somewhere else to complete screening.
Standard #: 22VAC40-185-280-B Description: Based on observation on 01/06/2017, the center did not ensure that hazardous substances such as cleaning materials, shall be kept in a locked place using a safe locking method that prevents access by children. EVIDENCE: 1. A container of Drano was in an unlocked cabinet in the bathroom of the Pink Room. 2. A spray bottle of degreaser was in an unlocked cabinet in the Music Room. Both cabinets are high and out of the reach of children. Plan of Correction: Corrected on site. Items removed from rooms. The rooms are shared space with another organization. We will remind them to remove items and lock cabinets.

EVIDENCE: A box of Alka Seltzer tablets were observed in an unlocked cabinet in the bathroom of the Pink Room. Plan of Correction: Corrected on site. Removed from classroom and cabinet locked.
