Standard #: 22VAC40-185-60-A Description: Based on review of children records the facility failed to maintain compliance with this standard as required relating to information for emergency contacts. Evidence: Child record #5 did not have the emergency contacts addresses as required. Child record #5 did not have a phone number for one emergency contact as required. The listed information had been updated since original enrollment and the updated emergency contact information was not complete. Plan of Correction: This information will be obtained from the guardian this afternoon at pick up. Administration will continue to review and update files for compliance. Staff will also be reminded that when entering child information into the system that addresses and phone numbers be entered for emergency contacts.
Standard #: 22VAC40-185-510-N Description: Based on review of medications the facility failed to maintain compliance with this standard as required. Evidence: The epi-pen for child A had expired as of 12/2016 and was still in the medication lock box for administration in an emergency. Plan of Correction: Administration has already requested renewal of the epi-pen from the parent. Administration will contact the parent again today to remind the parent of the original request, and to obtain a status update as to the replacement of the expired medication. Administration and staff will continue to monitor the medications for compliance.
