An unannounced monitoring inspection was completed on January 18, 2017. The purpose of this visit was to measure the facility?s compliance with most but not all of, the licensing standards. The following ratios were observed: Infants 1-7 children/2 staff; Infant/Toddler- 5 children/1 staff; Two year olds- 8 children/1 staff Preschool / school age- 18 children/2 staff. The infants were observed napping, being held and playing on the floor. The toddlers were playing with toys with the teacher. The twos were taking pictures for Picture Day. The preschool children were engaged in circle time. They were counting by tens, reviewing letter recognition, reciting the months of the year/days of the week and reciting nursery rhymes. The facility administers medication. Three staff have current MAT training. The last evacuation drill was conducted 12/27/16. The most recent shelter in place drill was conducted 9/9/16. Three children?s records were reviewed and new staff records were reviewed as well. Approximate time on site: 10:05am to 12:45pm
Standard #: 22VAC40-185-350-C Description: Based on observations in the infant/toddler classroom, the facility did not ensure that the proper ratio for a mixed age group of children was maintained. EVIDENCE: There were 12 children in the infant/ toddler classroom. Although there were three staff persons in the room, the two groups were separated by a half wall with a locked gate. One staff person had supervision responsibility of a group of five children, which included a 15 month old infant. The staff child ratio applicable to the youngest child applies to the entire group. In this case, the ratio for infants is 1:4. Plan of Correction: The child has been moved back to the infant side. The teacher and I will do assessments and speak with mom and request that she write a letter confirming her request that he be moved to the toddler side.
Standard #: 22VAC40-185-370-5 Description: Based on a review of the infant classroom, the facility did not ensure that prohibited equipment was not used. EVIDENCE: The inspector observed an infant in a walker. Walkers are not permitted in infant spaces. Plan of Correction: The walker has been removed and will not be used in the infant room anymore.
