LI Anderson conducted an unannounced monitor inspection on 1/11/2017. Inspector arrived at 2:55 PM and concluded inspection at 5:00 PM. Children arrived at 3:30. There were twelve (12) school age children present under the direct care & supervision of two (2) caregivers. Two (2) children & one (1) staff record were reviewed during today?s inspection. After school program administers medication however there were no children present receiving medication during the center?s hours of operation. Children were observed during arrival/departure procedures, directed/individual activities, gymnasium activities & afternoon snacks. The exit interview including review of the Risk Assessment and areas of non-compliance found on 1/11/2017 were reviewed with the After-School Coordinator on 1/11/2017.
Standard #: 22VAC40-185-260-B Description: Based on inspection, the Licensee failed to have annual approval from the health department. EVIDENCE: Annual health inspection was unavailable. Most recent health inspection was completed on 6/05/2015 Plan of Correction: The program director will ensure that the most recent health inspection is located in the ?inspection? notebook and available for view when necessary.
Standard #: 22VAC40-185-540-C Description: Based on inspection, the Licensee failed to have all required first aid supplies on bus used to transport children. EVIDENCE: The First Aid kit on Bus #3 did not include single use gloves. Plan of Correction: Use the first aid checklist to ensure all supplies are on all buses. Gloves were added to the first aid kit on bus #3 to be in compliance.
Standard #: 22VAC40-185-550-D Description: Based on record review & discussion with staff, the Licensee failed to implement a monthly practice evacuation drill and a minimum of two (2) shelter-in-place drills per year for the most likely to occur scenarios. EVIDENCE: Evacuation drill was not conducted in December 2016. At least two (2) shelter-in-place drills were not conducted during 2016. Plan of Correction: The center will conduct a monthly evacuation drill and ensure that such drills are recorded and maintained for 1 year. The center will conduct a minimum of 2 shelter in place drills per year and will ensure that such drills are recorded and maintained for 1 year.
Standard #: 22VAC40-185-580-C Description: Based on review of transportation records & discussion with staff, the Licensee failed to have a list of the names of the children being transported & documentation of emergency numbers as specified in 22VAC40-185-550 F and H for each child being transported. EVIDENCE: LI Anderson observed twelve (12) school age children departing from the Center's Bus. Daily attendance of children present during was not available. LI Anderson reviewed transportation records, however emergency contact information was unavailable for each child being transported. Plan of Correction: All children who are transported will have an emergency form in the van notebook located on each vehicle. A list of all children who are transported will be available and used by van drivers on a daily basis to document those children in attendance.
