An unannounced monitoring inspection was conducted during the aftercare portion of the program on January 19, 2017. There were staff present with 16 school age children. Children were observed doing homework, playing freely and doing multiple tabletop activities in the cafeteria to include board games, chess, puzzles and cards. The children were also observed handwashing and having snack which consisted of chili and apple juice. Three children's records were reviewed on this date. One medication was reviewed on this date. Licensing Inspector reviewed supervision, activities, equipment, hand washing, nutrition, injury report documentation, medication procedures, children's records, emergency evacuation and shelter-in-place drill documentation, fire and health inspection reports, liability insurance coverage, emergency supplies and required postings. Violations were reviewed with and verified by the Site Supervisor.
Standard #: 22VAC40-185-60-A Description: Based on record review, in two of three children's records reviewed, the licensee did not ensure that each child's record was maintained with all required information. Evidence: 1. During the inspection conducted on January 19, 2017, the record for Child #1 did not include documentation of the home phone number and work phone number for one parent. 2. The record for Child #3 did not include documentation of the work phone number for both parents. Plan of Correction: The Site Supervisor will contact the parents to obtain the information.
