Comments:
An unannounced monitoring inspection was conducted today, 1/20/17. The children involved in center play, reading books, playing blocks, dress up, and puppets. The children went outside to play on the playground. Three children's files reviewed. The staff were introduced to the new LI. If you have any questions please do not hesitate to call or e-mail me. Inspection began: 9:55 am ended 11:20 am.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on a tour of the physical plant, the facility did not maintain all areas in a safe condition. Evidence: 1. The climbing equipment located on the playground has a deck under the green canopy with a horizontal wooden board that is split where two bolts attach it to the wire deck. Also, on this deck are three bolts missing that should attach the board to the wire deck. 2. On the playground equipment there are 1/2 inch to 1 inch gaps where the slides attach to the decking of the climber. These gaps present entanglement hazards.

Plan of Correction: Education Supervisor will do a work order to have maintenance make the repairs to the equipment. Children will not be allowed on the equipment until repairs are complete.
