22VAC40-191 Background Checks (22VAC40-191)

Technical Assistance:
1)You will need to submit your up to date parent and employee handbooks to Licensing by 1/18/17. 2)Please follow the timely submission and follow up on background checks outlined under Background Checks for Child Welfare Agencies book standard 60C and 60D ( Explaining consequences of unsatisfactory background checks results). You will need new background checks on all staff whose current background checks were conducted more than 90 days prior to 10/7/16, which was when you submitted your initial application. 3) Please provide parents and employees with your new handbooks( policies/procedures outlined under standards 240B and 420A) upon employment and enrollment. 4) The parental agreements between the parent and center outlined under standard 90A (1-3) must be obtained prior to each child's first day. 5) A director qualified staff will need to provide orientation training (standard 240A 1-6) to all staff within 7 days. 6) Consultation provided on mulch, spacing of playground equipment and hazards on the playground.

Comments:
A scheduled initial study was conducted today with the new owner. This is an existing center that is in the process of changing ownership and operational responsibilities. Fifty eight children were present with 10 staff providing supervision. The children were engaged in a variety of classroom activities. There was ample supplies and furnishings for the requested capacity. Please submit your bill of purchase/sale as soon as possible. Once that has been received as well as the up to date handbooks your file will be submitted to my Administrator for review. Arrival time: 10 am Departure time: 1: 45 pm

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation on the kiddie playground, it was determined that the playground equipment was not maintained in a safe condition. Evidence: The hand grips on the rocker was protruding and presented an impalement hazard.

Plan of Correction: We will remove the rocker from the playground.

Standard #: 22VAC40-185-330-B
Description: Based on an inspection of the two playgrounds it was determined that there was insufficient resilient surfacing and inadequate fall zones. Evidence: On the big playground, the swing set required a back fall zone of 13 feet 9 inches but had a 9 feet 9 inch fall zone as the wooden border was in the fall zone of the swings. The metal slide was within 4 feet of the wooden border and not the required 6 feet. The mulch on the big and kiddie playgrounds measured 1 to 2 inches of depth under and around the slides, swings and merry go round and not the required 6 inches.

Plan of Correction: We will place all of this equipment off limits until brought into full compliance
