The following Monitoring Inspection took place between the hours of 9:05am and 11:15am which included the exit interview. Today, I met with Julie Ward, staff of the Logos Academy located in Herndon, Virginia. Phone conference also held with the Center's Director who was out of town. At the start of this unannounced monitoring inspection there were a total of 19 children with 4 staff; good ratios in all two rooms. A total of 5 children?s files and 5 staff files were reviewed. The children were engaged in the following: Singing spanish songs in spanish class and making snowman cottonball art projects, nice transitions were observed from one activity to another, staff observed encouraging one child who was discouraged with the use of scissors and other organized activities. Standard #: 22VAC40-185-280-B Description: Based on observation of physical plant, hazardous substances were found accessible to children. Evidence: The closet in the nursery room was unlocked and contained cleaning agents. Plan of Correction: Staff immediately removed the cleaning agents from the closet.
Standard #: 22VAC40-185-290-3 Description: Based on observation of physical plant, one electrical outlet did not have a protective cover. Evidence: The outlet in the nursery room beside the red couch was found without protective covers. Plan of Correction: Staff immediately placed protective covers in the outlet.
Standard #: 22VAC40-185-550-D Description: Based on review of monthly practice evacuation drills, the center did not conduct one required drill. Evidence: No documentation found of an evacuation drill for the month of May. Plan of Correction: Center Director will ensure that practice evacuation drills are documented monthly on a consistent basis.
