An unannounced monitoring inspection was conducted today, January 27, 2017 to determine compliance with licensing standards. The inspector arrived at 10:00 am and concluded the inspection at 12:30 pm. Upon the inspector arrival children were engaged in a variety of activities. Children were divided into three rooms. Classroom #1 was observed having snack and playing in centers. Children played with Polly pockets, blocks and puzzles. Staff sat at the table and worked with children and their art project of ink dot stamping. Classroom #2 has been studying the Solar System and shared with the inspector the names of the planets. Children also told the inspector facts about the star Pluto. Classroom #3 had story time and looked at seek and find books. Staff interacted with children in a positive manner and circulated around the room to spend time with each grouping of children. Children had reviewed the letter "S" and make a snake out of a "S" paper cut. Classrooms were equipped with age appropriate materials that were in operable condition. Children's artwork was displayed on the walls of the classrooms and hallway. Later children had lunch. Children provide their own meals. The lunch boxes were labeled with names and today's date. All areas of the facility to include classrooms, bathrooms, hallways, and playground were inspected. The facility had the license, last inspection notice, emergency numbers, emergency evacuation procedures/maps, daily schedule and various parent information posted. The first aid kit and the non-medical emergency supplies were inspected and found complete. The inspector interacted with staff and children in care. Five staff records (see violation notice) and three children's records were reviewed. Staff have current CPR, First Aid and daily health observation training. One staff has current MAT certification. Medications and authorizations were reviewed. The most recent evacuation drill was on 12/22/2016. The two required shelter-in-place drills for 2016 were completed on 04/18/2016 and 05/31/2016. Injury reports were reviewed. Today's Ratio's: Classroom #1: (2.5-3 years) 7 children with 1 staff. Classroom #2: (3-4 years) 6 children with 2 staff. Classroom #3: (3-4 years) 5 children with 1 staff. Fire Inspection: 06/29/2016 Health 02/04/2016 The violations from the previous inspection on July 22, 2016 were checked for corrections. Standard #: 22VAC40-185-70-A Description: Based on review of staff records, the facility did not obtain all required information for each staff record. Evidence: 1. Staff #2 did not have the address of a person to be notified in an emergency which was kept at the center. A name, address and phone number of an emergency contact is required. Staff #2 only had the name and phone number listed in the record. 2. Staff #4, employed as a program leader did not have written documentation to demonstrate that she possesses the education required by the job position. Staff #4 did not have written documentation of a high school diploma or GED. Plan of Correction: Per the Director: 1. I will have the staff complete the missing address for her emergency contact. 2. The staff contacted the county school system and they stated the records had been destroyed in a fire. I will have the staff get a letter that records were destroyed. I will see if she can get a copy of college transcript.
Standard #: 22VAC40-185-240-A Description: Based on review of staff records and interview, the facility did not ensure staff received orientation training by the end of their first day of assuming job responsibilities. Evidence: Staff #2, with a hire date of 09/06/2016, did not have documentation of receiving orientation training by the end of her first day of assuming job responsibilities. The Director stated that orientation had not been completed. Plan of Correction: Per the Director: I will complete the orientation training with the staff as soon as possible.
