- Technical Assistance:
The licensing inspector conducted an unannounced monitoring inspection Monday, January 30, 2017 from approximately 2:50pm to 5:10pm. The following ratios were observed with a total of 106 children present during the inspection:
Kinder Quest: 27 children with 5 staff
Cougar Quest: 58 children with 7 staff
Middle Quest: 21 children with 3 staff
During the inspection children were observed completing homework, reading, playing computer games, eating snack, playing outside, building with blocks, drawing, playing in the sensory table and playing with play-doh. The snack consisted of smoothies and graham crackers. Staff was observed providing assistance when needed and engaged in conversations with children. All areas of the facility including classrooms, hallways, bathrooms and playgrounds were inspected.
The following information was reviewed during the inspection: daily attendance, emergency drill logs, emergency supplies, emergency preparedness plan, parent agreements, and staff training information. All required items were posted. Medication is administered and medication and medication authorization forms were reviewed.

Standard #: 22VAC40-185-270-A Description: Based on observations, the facility did not maintain all equipment in a safe and operable condition.
During the inspection, the licensing inspector observed a water and sand table located on the KinderQuest playground that contained a cracked plastic container placed at the top of the table and a sharp jagged piece of white plastic located underneath the table.
Plan of Correction: Per management - The children will not use the table until maintenance can fix the broken items.
