- Technical Assistance:
Technical assistance was given regarding clear documentation of a complete address as required in children's records.
An unannounced monitoring inspection was conducted on January 30, 2017 from approximately 12:25 p.m. until 5:05 p.m.. At the time of entrance there were 47 children and 6 staff members present with children. There were additional staff on site for kitchen and administrative purposes. The sample size consisted of 5 children's records, 3 staff records and the background checks of two bard members. Four medications were reviewed. Children were observed during their rest period, preparing for and eating snack and playing with puzzles and puppets. Previous violations were reviewed. Areas of non-compliance are identified on the violation notice. The results of the inspection were reviewed and verified with the center director during the exit interview. Standard #: 22VAC40-185-40-I Description: Based on documentation review and staff interviews, the licensee did not ensure the center developed a written procedures for injury prevention. Evidence: During the inspection on January 30, 2017, there was not a written injury prevention plan available for review. Plan of Correction: The director will write a plan.
Standard #: 22VAC40-185-60-A Description: Based on a record review and staff interviews, the licensee did not ensure all required written informatory had been obtianed for the children enrolled. Evidence: During the inspection on January 30, 2017, the following records were determined to be incomplete; 1) The records for child #1, child #3, child #4, and child #5 did not include information on a) chronic physical problems and pertinent developmental information and any special accommodations needed b) previous child day care and schools attended by the child c) name of any additional programs or schools that the child is concurrently attending and the grade or class level 2) The record for child #1 and child # 4 did not include a house number or street name for one of the emergency contacts listed. Plan of Correction: The director will obtain information and amend the registration form.
Standard #: 22VAC40-185-500-A Description: Based on observation, the licensee did not ensure that children's hands were washed with soap and running water or disposable wipes before and after eating meals or snacks or staff members hands were washed as required. Evidence: During the inspection on January 30, 2017, in room 109 the staff and children were observed not washing their hands as required. 1) There were 7 children and all were observed eating snack and then immediately playing with puzzles and puppets. The children did not wash their hands after they eat snack. 2) Staff #6 was observed changing a diaper and after the diaper change was complete the staff member assisted other children with cleaning up from snack, cleaning their faces and putting a way toys and then washed their hands . The staff members hands must be washed immediately after changing diapers. Plan of Correction: The director will retrain the staff will model the required procedures.
Standard #: 22VAC40-185-550-L Description: Based on observation and staff interviews, the licensee did not ensure the center notified the parent by the end of the day of any known minor injuries with all required information. Evidence; During the inspection on January 30, 2017, 10 injury reports were reviewed and 3 did not contain the date, the time and how the parent was notified of the child's injury. Plan of Correction: The director will train staff and amend the form.
Standard #: 22VAC40-185-560-K Description: Based on observation, the licensee did not ensure tables were sanitized before and after each use for feeding. Evidence: During the inspection on January 30, 2017, the table in room 102 was not sanitized prior to using it for feeding. Plan of Correction: The director retrain the staff.
Standard #: 22VAC40-191-40-D-1-C Description: Based on a record review and staff interviews the licensee, did not ensure all individuals required to have a criminal history record check (CRC) before three years since the dates of the last CRC was conducted. Evidence: During the inspection on January 30, 2017, the most recent CRC for board member #1 was dated 2-13-2013. Plan of Correction: The director ensure all background checks are current.
Standard #: 22VAC40-191-60-C-1 Description: Based on observation and staff interviews, the licensee did not ensure employees were denied continued employment when an original criminal history record report (CRC) was not obtained within 30 days of employment. Evidence: During the inspection on January 30, 2017, there were two staff members observed working that have not obtained an original CRC. Staff #1 date of hire was 12-16-2016 and staff #2 date of hire was 12-19-2016. Plan of Correction: The director ensure all background checks are done and removed the staff from working with anything pertaining to the children or program.
