Comments:
The licensing inspector conducted an unannounced renewal inspection Friday, January 20, 2017 from 9:15am to 10:30am. The children were observed listening to stories, playing with toys and preparing for snack. The children bring their own snack and lunch from home. All classrooms and playgrounds were inspected today. Due to rainy weather the facility?s van was not inspected. Three children?s records and three employee records were reviewed during this inspection. Medication is being administered and was reviewed. A sample of ten of injury reports were reviewed. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 1/19/2017 Last shelter-in-place drill: 1/19/2017 Total enrollment: 44 Today, the following child to staff ratios were observed: Twos & Threes ? 7:1 Threes & Fours ? 8:1 School Age ? not present The violations from the previous inspections were checked for corrections. There was one repeat violation found during today?s inspection. The Department has not yet received a complete renewal application and annual fee from the center. The Department must receive a complete renewal application and fee prior to expiration of the license on March 1, 2017 or the Department cannot renew the license. Standard #: 22VAC40-185-500-B
Description: Based on observation on 01/20/2017, the center did not ensure that the surface used for diapering was nonabsorbent. EVIDENCE: The mat on the changing table in the boy's bathroom had a tear in the vinyl covering. The nonabsorbent foam padding was exposed.

Plan of Correction: Corrected on site. The mat was tapped to cover the tear.

Standard #: 22VAC40-185-520-A
Description: Based on observation on 01/20/2017 and staff interview, the center did not ensure that over-the-counter skin products shall not be kept beyond the expiration date of the product. EVIDENCE: A tube of petroleum jelly was observed on the diaper changing table in the boy's bathroom. Staff #2 stated that the product was the center's, and it was used on the children. The petroleum jelly expired on 12/2016.

Plan of Correction: Corrected on site. The product was discarded.

Standard #: 22VAC40-191-60-C-1
Description: Based on a review of staff records on 01/20/2017, the center did not ensure that each staff record contained an original criminal history record report within 30 days of employment. EVIDENCE: The earliest criminal history record report in the record of Staff #1, whose date of hire was 08/01/2016, is dated 12/5/2016.

Plan of Correction: From now on will submit background checks on employee's first day.
