**Discussed with the Director to keep an eye on the preschool double green slide. It is starting to separate from the platform causing a small gap which may become a hazard. Once that separation occurs please make sure it is corrected to avoid any type of accident or injury that it may cause.
An unannounced monitoring inspection was conducted today from 10:30am-12:50pm. There were 122 (infants-5yrs.) directly supervised by 21 staff. The physical plant, 2 staff records, 5 children?s records, evacuation drills, emergency supplies, and policies were inspected. Children were observed participating in fitness activities, circle time, group and individual projects and infants arriving for care. Hand-washing procedures were also observed. There was an abundant supply of books, toys, and materials for the children. The center was clean and organized. Areas of non-compliance are identified in this report. Standard #: 22VAC40-185-130-B Description: Based on review, the facility failed to ensure that children under 2 years of age must submit documentation of additional immunizations once every six months. Evidence: Reviewed 5 children's records and found that CH2's last documented immunization was on 1/14/16. Plan of Correction: I contacted the parent and left a message to have them faxed to us and will follow up.
Standard #: 22VAC40-185-280-B Description: Based on observation, the facility failed to ensure that hazardous substances are kept in a locked place using a safe locking method that prevents access by children. Evidence: Observed in an unlocked cabinet in the Preschool 2 Room and an unlocked closet in the Fitness Room with spray bottles containing soap water and bleach water. Plan of Correction: Will call to have them add a lock to the fitness room, and will remind teachers verbally now and during a staff meeting.
Standard #: 22VAC40-185-290-3 Description: Based on observation, the facility failed to ensure that unused electrical outlets have protective covers. Evidence: Observed 1 electrical outlet in the hallway near the Get Set room that was rotated to the "open" position. Plan of Correction: Corrected during the inspection.
