Comments:
An unannounced monitoring inspection was conducted on February 1, 2017. There were 37 children present with six staff members and a volunteer in three classrooms. The children were observed during outdoor play, practicing positive affirmation, doing a group letter recognition activity on the computer and napping. Two medications were reviewed during this inspection. Three children's records were reviewed during the inspection.

Violations:
Standard #: 22VAC40-185-140-A
Description: Based on record review, in one of three children's records reviewed, the licensee did not ensure that each child shall have a physical examination by or under the direction of a physician before the child's attendance or within one after attendance. Evidence: During the inspection conducted on February 1, 2017, there was no documentation of a physical examination in the record for Child #3 (start date as indicated in the record 12/12/16).

Plan of Correction: Lead Teacher will ask parents to submit the information.

Standard #: 22VAC40-185-60-A
Description: Based on record review, in three of three children's records reviewed, the licensee did not ensure that each child's record was maintained with all required information. Evidence: 1. During the inspection conducted on February 1, 2017, the record for Child #1 did not include documentation of the following: name of any additional programs or schools that the child is concurrently attending and the grade or class level; viewing proof of the child's identity; home phone number for both parents. 2. The record for Child #2 did not include documentation of the following: name of any additional programs or schools that the child is concurrently attending and the grade or class level; viewing proof of the child's identity; work name and phone number for one parent and the work phone number for the other parent. 3. The record for Child #3 did not include documentation of the name of any additional programs or schools that the child is concurrently attending and the grade or class level and viewing proof of the child's identity.

Plan of Correction: Lead Teacher will ask parents to submit the information.

Standard #: 22VAC40-185-270-A
Description: Based on observation and inspection of the facility, the licensee did not ensure that areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Unsafe conditions shall include, but not be limited to, splintered, cracked or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting or breakage of any equipment; head entrapment hazards; and protruding nails, bolts or other components that could entangle clothing or snag skin. Evidence: During the inspection conducted on February 1, 2017, the licensing inspector observed a broken toilet paper dispenser in the PC 2 classroom that had become detached from the wall with an exposed screw and crumbling drywall underneath.

Plan of Correction: A work order has already been submitted. The work order has been sent to Administrative staff of the school, the City of Hampton's maintenance department, the ECE Director and the Director of the center.

Standard #: 22VAC40-185-280-B
Description: Based on observation and inspection of the facility, the staff members did not ensure that hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children. Evidence: During the inspection conducted on February 1, 2017, in the PC 3 classroom, the licensing inspector observed a bottle of hand sanitizer on the desk and a can of shaving cream on the shelf of a cubby. All items were labeled keep out of reach of children and caution and/or warning.

Plan of Correction: Items will be removed immediately. They will be secured and stored in a locked area.

Standard #: 22VAC40-185-540-D
Description: Based on observation and inspection of the facility, the licensee did not ensure that all required emergency supplies were at the center. Evidence: During the inspection conducted on February 1, 2017, the activated charcoal preparation for the center expired in January 2016.

Plan of Correction: Item has been ordered and waiting for it to come in. If it is not received we will check with local pharmacy.

Standard #: 22VAC40-185-560-F
Description: Based on observation and inspection of the facility, the licensee did not ensure that when centers choose to provide meals or snacks all requirements are followed. Evidence: During the inspection conducted on February 1, 2017, there was not a menu posted for the current one week period.

Plan of Correction: Corrected during the inspection.
