Comments:
An unannounced renewal inspection was conducted on 01/31/2017 from 10:45am - 1:35pm. At the time of entrance there were 40 preschool children in care with five staff. Children were observed during morning program time and outdoor play. Four children's records, seven staff records and six medication reviews were conducted. The violations are identified on the violation notice and were discussed with the director at the conclusion of the inspection.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation, the center failed to ensure that areas and equipment of the center shall be maintained in a safe condition. Evidence: The telephone cord from the wall mounted phone in room #9 was dangling and accessible. Dangling cords are an entanglement hazard.

Plan of Correction: The telephone cord in the preschool classroom was covered by a plastic protection strip. The coiled cord was lifted up using a hook attached to the adjacent cabinet.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the center failed to ensure that hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children. Evidence: A container of sanitizing wipes was stored, on a high shelf, inside of an unlocked cabinet in room #8b. The label on this product indicated the contents as a hazardous chemical.

Plan of Correction: The container of sanitizing wipes was moved to an adjacent locked cabinet. We are installing an additional slide lock on the other metal storage cabinet in that same classroom as well. Lock to be installed 2/3/17.

Standard #: 22VAC40-185-560-G
Description: Based on observation, the center failed to ensure that when food is brought from home the food container is clearly dated. Evidence: Three of four lunch containers viewed were not dated.

Plan of Correction: We have developed a "tag" and "date" system for all preschool lunches.
