Discussed with staff: 1. Driver who is working as only a driver and also drives for other Club activities. 2. Driver Statement 3. Parent leaving children at front door before the program is open. Doors were locked. 4, Allowing a non registered child into the program while she waits for her bus. 5. Accident reports are to be reviewed monthly to look for a pattern. Do not file before doing this.
An unannounced monitoring inspection was conducted beginning at 6:00 and ending at 7:15. By 7:10, there were 20 children with 4 staff. Children were observed playing video games, basketball and games in the gym and Homework at the tables. Reviewed 7 children's files - see violation notice Reviewed 4 staff files - in compliance Emergency escape drills and shelter in place drills are up to date. Standard #: 22VAC40-185-140-A Description: Based on a review of 7 children's files, it was determined that 1 child did not have a physical on file. Evidence : There was no physical on file for Child #1, who was enrolled on 8/26/16. Plan of Correction: Parent will be notified by staff and given a date to bring in the physical form.
Standard #: 22VAC40-185-340-A Description: Based on interview with staff, it was determined that a child in care did not have an emergency medication that was listed on the child's physical and permission forms. Evidence: Child #2, has medication permission forms on file for an Epipen and an inhaler. According to Morning staff, there is no medication at the facility. Staff cannot care for the child's medical needs as prescribed by the physician. Plan of Correction: Staff will contact the parent today. Parent will document in writing, if the child no longer needs these emergency medications.
